Job summary

Cleveland Clinic Fairview Hospital is seeking a Care Coordinator RN to collaborate with a multidisciplinary team across the continuum of care for high-risk patients. You will coordinate care, provide disease management, and focus on outreach to improve outcomes and reduce readmissions.

The role requires RN licensure, BLS certification, and 3–5 years of nursing experience. The position operates primarily 7:30am–4:30pm, with duties spanning Fairview and floating to other facilities.

Qualifications

• Graduate from an accredited nursing program with RN licensure.
• Current state RN license.
• BLS certification through AHA or Red Cross.
• Three to five years of nursing experience.
• Oncology-specific certification or ongoing qualifying education for oncology settings in a Cleveland Clinic facility.

Responsibilities

• Coordinate care and disease management for high-risk patients.
• Outreach to patients to promote wellness and reduce readmissions.
• Collaborate with multidisciplinary care teams across the continuum of care.
• Conduct comprehensive clinical assessments including disease, age, medical, behavioral, and social needs.
• Educate patients and families, monitor adherence, and update care plans.
