Job summary

Trinity Health Mid-Atlantic is seeking an RN Care Manager in Allendale for a full-time day shift. The role is on-site with one remote day per week, embedded in the practice with a supportive team.

The RN will coordinate care for patients with chronic conditions, conduct assessments, educate patients, and strive to improve outcomes, reduce costs, and promote health equity. Must have Michigan RN licensure and a BSN; MSN preferred.

Qualifications

• Graduate of an accredited school of nursing.
• BSN required.
• MSN preferred or actively pursuing.
• Current RN licensure in Michigan.
• 1 year of case management experience preferred; 3 years of nursing required.

Responsibilities

• Assess and plan ongoing care for patients with chronic conditions.
• Coordinate care across settings and providers to optimize outcomes.
• Engage patients/families with education and self-management plans.
• Monitor outcomes and manage risk stratification for populations.
• Support population health management and health equity initiatives.
