Job summary

Prominence Health, a value-based care organization, seeks an RN Care Manager to support medically and socially complex members under the Care Management Clinical Program Manager. Role focuses on person-centered, culturally sensitive care coordination to improve outcomes and independence.

Responsibilities include creating individualized care plans, coordinating with physicians and community resources, and ensuring alignment with CMSA standards of practice for case management within the SNP CM

Qualifications

• Associate or Bachelor’s Degree in Nursing, required.
• Active, unrestricted Compact RN license in applicable states.
• Minimum of three (3) years in clinical nursing practice, required.
• Minimum of three (3) years in Case Management/Transition of Care, preferred.
• Ability to communicate effectively in English; bilingual in Spanish/other is a plus.

Responsibilities

• Provide care management services for medically and socially complex members.
• Develop comprehensive care plans, including motivational assessment and end-of-life planning.
• Promote the Care Management program to contracted physicians and members; build community partnerships.
