Job summary

Baylor Scott & White Institute for Rehabilitation in Dallas, TX seeks a Full-Time Case Manager (RN, LMSW, PT, OT, SLP) for neuro rehabilitation. You will coordinate care with a transdisciplinary team, manage timelines, and oversee authorizations and discharge planning to achieve patient outcomes.

Qualifications include a clinical license, BLS, a Bachelor’s degree with 2+ years in case management, and CCM certification preferred. A supportive benefits package is offered.

Qualifications

• Current licensure in a clinical discipline per state guidelines (RN, LMSW preferred).
• BLS certification through the American Heart Association required.
• State Driver's License in good standing required.
• Bachelor's Degree + two years experience in case management or neurorehabilitation preferred; CCM preferable.

Responsibilities

• Coordinate communication within the transdisciplinary team and with patients and families.
• Monitor program delivery to meet patient outcomes and provide resource support.
• Lead conferences to review progress and modify care plans as needed.
• Oversee insurance authorizations and ensure accurate billing in the EHR.
• Coordinate discharge planning and follow-up services with families.
