Job summary

Baylor Scott & White Health is seeking a Transfer Coordinator RN to join the Transfer Center in Temple, TX. This onsite role requires a strong critical care background (ICU/ED/PACU) and ensures safe, efficient patient movement within the health system.

The RN will coordinate communications among referring physicians, hospitals, patients, and internal departments, applying clinical judgment and care coordination to optimize placements. Onsite daytime shifts with rotating weekends.

Qualifications

• Associate's degree in nursing is required.
• Three years of work experience minimum.
• RN license must be valid and current.

Responsibilities

• Coordinate patient transfers across the Baylor Scott & White Health system with physicians, nurses, hospitals, and departments.
• Review clinical information from referring hospitals to support appropriate placement.
• Apply nursing knowledge and clinical judgment to transfers and resource access.
• Ensure documentation complies with laws, standards, and policies.
• Provide excellent customer service to referring physicians, organizations, patients, families, and internal teams.
• Communicate clearly with stakeholders throughout the transfer process.
• Oversee transfer center operations and prioritize multiple requests in a fast-paced environment.
