Job summary

Corewell Health is seeking a part-time RN Case Manager at Blodgett Hospital in Grand Rapids. You will plan, coordinate, and evaluate patient care with a focus on managing caseloads and promoting population health.

Collaboration with PCPs, specialists, and payers is essential to improve access and outcomes. The role requires a BSN or higher and Michigan RN licensure, with 2–5+ years in nursing/case management.

Qualifications

• Bachelor's degree or higher in nursing or a health-related field.
• BSN must be completed within 7 years for newer hires.
• Current RN license in Michigan is required.
• CCM certification preferred upon hire.

Responsibilities

• Plans and prioritizes care for individuals and populations to promote optimal health.
• Assesses patient needs, coordinates with PCPs and specialists to manage care.
• Develops care plans, educates patients/families, and documents outcomes.
• Communicates with providers, payers, and team to improve access and continuity.
• Measures outcomes and analyzes data trends for groups of patients.

Education

Bachelor's Degree or equivalent in Nursing or healthcare-related field

BSN must be obtained within 7 years (start date on/after Jan 1, 2017)

RN licensed in Michigan

CCM certification (Upon Hire preferred)
