Job summary

Corewell Health in Grand Rapids is seeking an experienced Registered Nurse for Care Management to plan and prioritize care for individuals and populations, focusing on strategies that promote optimal health within communities. You will collaborate with PCPs, specialists and payers to coordinate services and measure outcomes.

The role requires a BSN within 7 years and 2–5 years of relevant clinical nursing and/or case management experience, with a comprehensive benefits package and on‑demand pay

Qualifications

• Bachelor's degree in nursing or equivalent; BSN must be obtained within 7 years for Corewell Health Hospitals.
• 2 years of relevant clinical nursing and/or case management experience.
• 5 years of relevant clinical nursing and/or case management experience.

Responsibilities

• Assess referrals to identify patient needs, intensity, benefits and resources.
• Develop plans of care and coordinate with PCPs, specialists and payers.
• Educate patients and families; implement teaching strategies and referrals.
• Coordinate services to improve access and promote cost-effective care.
• Document data, plans, interventions and outcomes per guidelines.
