Job summary

Qualifications

• Bachelor's Degree or equivalent nursing or a healthcare related field.
• BSN must be obtained within 7 years for those who start on or after Jan 1, 2017.
• 2 years of relevant clinical nursing and/or case management/managed care or related field preferred.

Responsibilities

• Plans and prioritizes care for individuals and populations to promote optimal health.
• Develops plan of care and coordinates with PCPs, specialists, and other health team members.
• Documents patient data, interventions and outcomes per department guidelines.
