Job summary

The Clinic @ Central Oklahoma Family Medical Center is seeking an RN Care Manager to coordinate medical treatment, assess and develop care plans, and monitor medication compliance for assigned patients. You will connect patients with social supports and support regulatory compliance and PCMH standards.

Responsibilities include analyzing social determinants of health, leading improvement activities, coordinating with external referrals, and supporting quality outcomes through care management.

Qualifications

• Current licensure as a Registered Nurse in Oklahoma.
• BSN or higher preferred; CMGT BC or related field with 5+ years of experience.
• Minimum of 3 years bedside nursing experience; care management/population health experience preferred; proven leadership and communication skills.

Responsibilities

• Aids departments and senior management in a professional and timely fashion.
• Improves care management processes with consultative services that assure proactive approach.
• Oversees the care management model.
• Analyzes social determinants of health for populations served.
• Monitors and leads improvement activities to connect resources with patient needs.
• Leads initiatives to build health equity for populations served.
• Coordinates external referral sources.
• Ensures appropriate utilization of healthcare resources to maintain PCMH certification.
• Assists with annual wellness visits (AWV).
• Develops data for CM initiatives; identifies EHR efficiencies.
• Collaborates to improve preventive screening rates.
• Supports quality and efficiency outcomes through care coordination.
