Job summary

OU Health is seeking a Care Management Registered Nurse for a full-time float position in Oklahoma City. The role coordinates care for selected patient populations, ensures efficient resource use, and supports quality outcomes while maintaining patient satisfaction.

Responsibilities include conducting comprehensive assessments, developing care plans with the care team, educating patients and families, coordinating with providers and payers, and evaluating care effectiveness.

Qualifications

• Associate degree or higher in nursing required.
• RN license (OK State Board or eNLC) required.
• BLS from AHA required.
• Experience in care management preferred.

Responsibilities

• Coordinate and facilitate patient care across teams.
• Assess patient health status and plan individualized care.
• Educate patients and families about conditions, plans, medications.
• Monitor and optimize care plans for outcomes and cost.
• Ensure compliance with regulations and quality standards.
