Job summary

OU Health in Downtown Oklahoma City is seeking a Complex Care Manager, RN for Day shift in the Emergency Department. You will coordinate care for high-risk patients, review EMRs, and collaborate with an interdisciplinary team to close gaps in care.

The role focuses on reducing readmissions, optimizing transitions of care, and advocating for patient-centered services within CMS guidelines.

Qualifications

• RN with BSN or Social Work path: LMSW/LCSW; BLS required.
• RN license in OK or eNLC; active license.
• LMSW/LCSW license under supervision acceptable.

Responsibilities

• Conducts EMR reviews and patient interviews to assess and close care gaps.
• Performs readmission reviews to reduce avoidable hospitalizations.
• Manages denial cases and advocates for appropriate patient care coverage.
• Facilitates complex discharge planning for safe transitions of care.
• Analyzes utilization metrics and supports performance improvement.
• Collaborates with physicians and care teams to coordinate services.
