Job summary

MaineHealth is seeking a dedicated Registered Nurse to join the Ambulatory Care Management team in Bath, ME. This full-time role focuses on nursing assessment, care planning, patient support, and coordination within the primary care setting.

You’ll work Monday–Friday days, with no weekends, supporting patients across our medical group practices. The role emphasizes collaboration with the care team to ensure timely care delivery, quality outcomes, and patient-centered planning.

Qualifications

• Education: Graduate of an accredited School of Nursing required; Bachelor’s Degree preferred.
• License/Certifications: Current state RN license; BLS certification required or obtained within 30 days.
• Experience: Two years of RN experience preferred.

Responsibilities

• Perform nursing assessment, develop care plans, and coordinate ambulatory care.
• Provide patient support and education, and communicate with the care team to ensure continuity of care.
• Collaborate with primary care teams to optimize patient outcomes and facilitate care coordination.
• Maintain adherence to scope of practice and related policies.
