MaineHealth is hiring a Registered Nurse - Care Manager - Ambulatory - MCH in a Monday–Friday day-shift role without weekends. You will partner with the primary care team to perform nursing assessments, plan care, support patients, and coordinate services to ensure seamless care.

Qualifications

• Education: Graduate of an accredited School of Nursing required; Bachelor's Degree preferred.
• License/Certifications: Current RN license; BLS required or obtained within 30 days; anticoagulation certification within one year.
• Experience: Two years of RN experience preferred.

Responsibilities

• Perform nursing assessment, care planning and implementation.
• Act as liaison with the care team to coordinate care.
• Provide patient support and care coordination in ambulatory settings.
