HealthSource of Ohio is seeking a Care Manager- Registered Nurse for its Batavia, OH site. This full-time role focuses on guiding patients through the healthcare system, coordinating care after hospital events, and educating families to reduce ER visits.

You will help develop Chronic Care Plans and participate in care teams. The position requires Ohio RN licensure with 2–3 years of outpatient RN experience and skills in data analysis and health care operations.

Qualifications

• RN licensure in Ohio is required.
• 2–3 years of clinical RN experience.
• Experience in outpatient settings as an RN.
• Experience with data analysis and health care operations.

Responsibilities

• Act as a patient advocate and navigator through the healthcare system.
• Coordinate continuity of patient care with patients and families after hospital events.
• Educate patients and families on reducing ER visits in the future.
• Create and support Chronic Care Plans.
• Participate in Patient-Centered Medical Home team meetings and quality improvement initiatives.
• Evaluate clinical care, utilization of resources, and develop tools and procedures.
