Qualifications

• 1 year home health, mental health, substance use, hospice, utilization review, discharge planning or care management experience.

Responsibilities

• Screen for post-acute needs and plan transitions.
• Coordinate care and educate patients and families.
• Refer to community care resources as needed.
• Develop care plans aligned with regulatory requirements and team.
• Monitor and reassess the care plan effectiveness.
