Job summary

Kaiser Permanente is seeking an Inpatient Care Manager (RN) to independently assess and coordinate transitions of care in acute and emergency settings, ensuring appropriate level of care and accurate billing. You will serve as expert consultant to physicians and care teams, develop care plans, and facilitate safe, timely discharges across settings.

Qualifications include RN licensure in Oregon within 6 months, BSN preferred, and strong communication, documentation, and care coordination skills

Qualifications

• RN program completed by date of hire.
• HS diploma or GED required.
• Valid RN license in Oregon within 6 months of hire or a multi-state compact license.
• Current Basic Life Support certification.

Responsibilities

• Coordinate post-discharge care to ensure timely, safe hospital discharges.
• Perform and document patient assessments appropriate to age and needs.
• Collaborate with patients and teams to develop care plans optimizing resources.
• Resolve barriers to safe transfers via interdisciplinary conferences.
• Maintain ongoing contact with staff and services for smooth transitions.
• Ensure regulatory standards are met and support care quality.
