Job summary

Providence St. Vincent Medical Center is seeking an RN Care Manager for a Full-Time day shift (1.0 FTE) with 5 eight-hour shifts weekly and every 4th weekend, 0800-1630.

The role involves screening for post-acute needs, transition planning, and coordinating care to progress patients along the continuum. Ideal candidates hold an Oregon RN license, have 2+ years acute care or community-based experience, and are proficient with electronic medical records; a BSN or higher is preferred.

Qualifications

• Nursing degree/diploma required upon hire.
• Bachelor's degree in Nursing required within 3 years of hire.
• Oregon Registered Nurse license required upon hire.
• Two years acute care hospital or community-based experience.
• One year experience with electronic medical records or documentation systems.
• One year home health, mental health, substance use, hospice, utilization review, discharge planning or care management experience.

Responsibilities

• Screen for post-acute needs and transition planning.
• Coordinate care and support discharge planning.
• Provide education and facilitate community care referrals.
