Providence Portland Medical Center in Portland is seeking an RN Care Manager for the acute care setting. This role delivers screening for post-acute needs, transition planning, care coordination and patient education to progress patients through the continuum of care. Full-time, day shift.
Requires nursing degree, Oregon RN license, 2 years acute care or community experience, EMR experience, and knowledge of discharge planning. CCM certification preferred.
