Altera Health & Rehabilitation Center in Fairhope, Alabama seeks an experienced RN Care Manager to lead care coordination from admission through discharge for short-term rehab and long-term residents.

You will collaborate with physicians, nursing, therapy, social services, and admissions to ensure timely documentation, regulatory compliance, and quality outcomes while guiding transitions of care.

Formación

• Current Alabama RN license required.
• Experience in skilled nursing, rehab or long-term care preferred.
• Strong clinical assessment, communication, and organizational skills.
• Ability to manage multiple priorities with an interdisciplinary team.

RN Care Manager (Registered Nurse) Fairhope, Alabama Altera Health & Rehabilitation Center

Altera Health & Rehabilitation Center is seeking a compassionate and experienced RN Care Manager to join our clinical leadership team in Fairhope, Alabama.

In this role, you will coordinate resident care from admission through discharge, collaborate with the interdisciplinary team, and support quality outcomes for our short-term rehabilitation and long-term care residents.

What You’ll Do

• Coordinate resident care from admission through discharge
• Collaborate with physicians, nursing, therapy, admissions, and social services
• Lead interdisciplinary care plan meetings
• Assist with discharge planning and transitions of care
• Coordinate care for Medicare, managed care, and long-term care residents
• Communicate with residents and families about care plans and progress
• Maintain accurate, timely clinical documentation and regulatory compliance
• Support quality improvement initiatives and help reduce avoidable hospital readmissions

Qualifications

• Current Alabama Registered Nurse (RN) license required
• Experience in skilled nursing, rehabilitation, long-term care, case management, MDS, admissions, or care coordination preferred
• Strong clinical assessment, communication, and organizational skills
• Ability to manage multiple priorities and work collaboratively with an interdisciplinary team
