Job summary

Altera Health & Rehabilitation Center in Fairhope, Alabama is seeking an experienced RN Care Manager to join our clinical leadership team and coordinate resident care from admission through discharge.

As part of the interdisciplinary team you will lead care plan meetings, collaborate with physicians, nursing, therapy, admissions, and social services, and support quality outcomes for our short-term and long-term residents while ensuring regulatory compliance.

Qualifications

• Current Alabama RN license required.
• Experience in skilled nursing, rehabilitation, long-term care, case management, MDS, admissions, or care coordination preferred.
• Strong clinical assessment, communication and organizational skills.
• Ability to manage multiple priorities with an interdisciplinary team.

Responsibilities

• Coordinate resident care from admission through discharge.
• Lead interdisciplinary care plan meetings with the team.
• Assist with discharge planning and transitions of care.
• Coordinate care for Medicare, managed care, and long-term care residents.
• Communicate with residents and families about care plans and progress.
• Maintain accurate, timely clinical documentation and regulatory compliance.
• Support quality improvement initiatives and help reduce avoidable hospital readmissions.
