Regions Hospital in Saint Paul, MN seeks a Hospital Care Management Supervisor to lead a multidisciplinary team, provide education on psychosocial aspects of care, and ensure efficient daily operations. You will mentor staff, perform psychosocial assessments, and coordinate care plans and referrals for patients and families.
The role requires nursing or social work credentials with licensure (RN in MN and LICSW) and 5 years of relevant experience. Leadership experience is preferred.

As an integral member of the interdisciplinary healthcare team, this position contributes to collaborative patient care and offers education to healthcare professionals on the psychosocial aspects of treatment. Additional duties may be assigned as needed.
This position will support the department manager in the planning, development, coordination, and oversight of services within the assigned department. The role is responsible for ensuring the department operates efficiently on a day-to-day basis while providing leadership, guidance, and consultation to program staff.
FTE: 1.0 (40 hrs. weekly), Day Shift. Monday - Friday, some weekends on-call.
Bachelor’s Degree in Nursing and/OR Master’s in Social Work.
Case Management Certification preferred
If LICSW, strongly prefer ability to provide Clinical Supervision to both BSW and MSW Social Workers, licensed through the Board of Social Work
