Job summary

Regions Hospital in Saint Paul, MN seeks a Hospital Care Management Supervisor to lead a multidisciplinary team, provide education on psychosocial aspects of care, and ensure efficient daily operations. You will mentor staff, perform psychosocial assessments, and coordinate care plans and referrals for patients and families.

The role requires nursing or social work credentials with licensure (RN in MN and LICSW) and 5 years of relevant experience. Leadership experience is preferred.

Qualifications

• Bachelor’s Degree in Nursing or Master’s in Social Work required.
• 5 years’ experience in your profession.
• RN licensure in Minnesota and/ or LICSW credential.
• Prior leadership or preceptorship experience preferred.

Responsibilities

• Mentor and coach staff, support department leaders, and provide performance feedback.
• Conduct clinical and psychosocial assessments, develop care plans, and refer patients and families as needed.
• Utilize a broad range of clinical approaches to support patient care and team education.
• Oversee day-to-day department operations and provide guidance and consultation to program staff.
