Job summary

Physicians Regional-Collier is seeking a Director of Care Management to lead planning, coordination, and oversight of care management services, including utilization review, discharge planning, and transitions of care. You will supervise staff and collaborate with physicians, nurses, payors, and other healthcare professionals to improve outcomes and optimize resource use.

Qualifications

• Associate Degree in Nursing or Master’s in Social Work required.
• 2–4 years of related field experience required.
• 2–4 years of leadership experience preferred.
• RN licensure or SW licensure required (as applicable).

Responsibilities

• Oversee daily operations of the Care Management department.
• Oversee utilization review and discharge planning processes.
• Collaborate with physicians, nurses, payors to optimize care.
• Monitor regulatory compliance and quality initiatives.
• Prepare reports on utilization and performance.
• Provide staff mentorship and performance evaluations.
