Job summary

Allegheny Health Network is seeking an experienced nurse to coordinate patient progress toward quality and cost outcomes. The role mobilizes resources and manages the system in response to variances, following established standards.

You will collaborate with physicians and multidisciplinary teams to ensure resources support patients' individualized goals and timely care, and you will implement an effective discharge plan to optimize transitions.

Qualifications

• BA/BS degree and Nursing school diploma required.
• Current State of PA RN licensure OR current multi-state licensure through the enhanced Nurse Licensure Compact (eNLC).
• 5 years' acute Medical/Surgical and/or specialty care nursing required.
• Preferred: Experience in utilization management and/or employment by a third party payer/review agency.

Responsibilities

• Coordinates patient progress toward achieving quality and cost outcomes; mobilizes resources and manages the system in response to concurrent variances according to the preestablished standards.
• Utilizes management practices and collaborates with physicians and multidisciplinary teams to ensure appropriate management of resources to attain patients' individualized goals.
• Implements an effective discharge plan.
