Job summary

Qualifications

Responsibilities

• Assesses, develops, and administers plan of care, completes documentation, and ensures compliance with regulatory agencies.
• Communicates with attending physician regarding plan of care and involves physician in clinical decisions.
• Assesses learning needs of patient and family and provides education.
• Coordinates patient care with patient, family, and interdisciplinary team.
• Coordinates administrative functions for the program, including staffing or marketing activities.
• Participates in performance improvement activities and other duties as assigned.
