Qualifications

• Associate's Degree in Nursing and a valid RN license (state or multi-state compact).

• Current BLS certification; additional certifications may be required.

Responsibilities

• Conduct psychosocial, functional, and medical assessments to identify care needs.
• Develop and manage individualized discharge plans, addressing barriers and ensuring team consensus.
• Collaborate with physicians, care teams, and social services to facilitate timely and appropriate care transitions.
• Identify and resolve delays in diagnostic and treatment processes.
• Document assessments, care plans, and interactions accurately in medical records.
