Qualifications

• Graduate of an accredited nursing program; 2+ years of acute hospital clinical experience.
• WA RN license required; CPR within 90 days of hire.
• Masters in Nursing or Case Management strongly preferred.

Responsibilities

• Completes and documents discharge planning assessments and updates the plan.
• Leads multidisciplinary discharge planning with patient, family, and post-acute providers.
• Oversees implementation of discharge plan and coordinates care utilization with the team.
• Identifies readmission risk and implements 30-day mitigation strategies.
