Job summary

Longmont United Hospital, part of the CommonSpirit Health family, seeks an RN Care Coordinator in Colorado to oversee care progression and discharge planning. You will coordinate with physicians, nursing, and postacute providers to ensure safe, efficient transitions and advocate for patients to improve outcomes.

The role requires an RN with strong clinical judgment and communication skills, plus knowledge of care coordination principles and cost-effective care delivery.

Qualifications

• Graduate of an accredited school of nursing.
• Minimum two years of acute hospital clinical experience or a Master’s degree in Case Management or Nursing in lieu of 1 year experience.
• RN license in required states including CO or Compact.
• BLS certification within 3 months of hire.

Responsibilities

• Oversee multidisciplinary discharge planning, engaging care teams, patient, and postacute providers.
• Coordinate, communicate, and collaborate with utilization management, nursing, physicians, and insurers to progress care.
• Advocate for patients and families, addressing barriers to transitions and improving patient experience.
• Identify readmission risks and implement mitigation strategies for at least 30 days.
• Oversee discharge plan implementation and progress through evaluation.
• Use evidence-based guidelines to ensure appropriate inpatient resource use.
