Join Cleveland Clinic Fairview Hospital as an RN Care Coordinator for Neurology. You will collaborate with a multidisciplinary team to coordinate care for high-risk patients, focusing on outreach, assessment, and timely treatment across the continuum of care.

The role is part-time, with shifts 8:00am-4:30pm or 8:30am-5:00pm, two days one week and three days the next, without weekends or on-call requirements. Experience in nursing and care coordination is essential; BSN is preferred.

Qualifications

• Graduate from an accredited school of Professional Nursing.
• Current RN licensure in the state.
• Basic Life Support (BLS) certification.
• Three to five years of nursing experience.
• BSN preferred.

Responsibilities

• Identify patients with ongoing care coordination needs and outline involvement.
• Conduct outreach to high-risk patients to ensure timely care.
• Use registries and lists to manage populations and assess needs.
• Inform patients and families and collaborate with the care team.
• Perform reassessments and update the plan of care.
• Close care gaps around chronic disease or surgical episodes.
• Serve as primary patient contact and liaison between patients, families, and staff.
• Assist in managing transitions of care and identify barriers.
• Partner with care teams across disciplines.
• Define and ensure disease-specific care paths.
• Assess health literacy and readiness to change; use teach-back.
• Coach patients on self-management; set goals.
• Educate about managing conditions across pre/postoperative stages.
