Job summary

Working full-time day shift, you will develop care plans, perform comprehensive clinical assessments, identify barriers, and connect patients with community resources while advocating for patients and families.

Qualifications

• Graduate from an accredited school of Professional Nursing.
• Current RN licensure in the state.
• BLS Certification required.
• 3-5 years of nursing experience.
• BSN preferred.

Responsibilities

• Work collaboratively with a multidisciplinary care team to develop goals and plan interventions.
• Provide care and disease management coordination.
• Identify patients with ongoing coordination needs and conduct targeted outreach.
• Conduct comprehensive clinical assessments including medical, behavioral, and social needs.
• Inform and work with patients and families regarding care coordination.
• Monitor patient compliance with care plans and perform reassessments.
• Serve as a liaison and advocate for patients and families.
• Assist in managing transitions of care across settings.
• Identify barriers, facilitate solutions, and connect to community resources.
