Job summary

Allegheny Health Network seeks a care coordination professional to create, implement, and evaluate patient care plans across the health continuum. You will collaborate with providers, clinical teams, health plans, and partners to optimize outcomes and engage patients and families.

You will coordinate care, advocate for patients, educate families on resources and transition planning, and contribute to reducing readmissions and ED utilization. Strong communication and teamwork are essential.

Qualifications

• Minimum Bachelor’s Degree in Nursing or equivalent; 3 years in a clinical nursing role preferred.
• Current PA RN licensure or multi-state licensure via eNLC.
• Knowledge of case management standards and transition planning.

Responsibilities

• Contributes to comprehensive assessment of patients and families.
• Identify barriers and analyze risk for admission/readmission.
• Coordinate clinical care with patient, family, and interdisciplinary team.
• Advocate for patients and families with providers, payers, and post-acute partners.
• Educate patients and families on care, utilization, and resources.
• Pursue professional development and adhere to hospital policies.
