As an RN Care Coordinator, you will collaborate with multidisciplinary care teams across the continuum of care for high-risk patients. The Care Coordinator manages care and disease management for patients with chronic conditions or surgical episodes, focusing on patient outreach and coordination to achieve optimal outcomes, promote wellness, reduce preventable ED visits and readmissions, and improve patient satisfaction.

Responsibilities:
  1. Identify patients with ongoing care coordination needs within the specialty practice.
  2. Define the scope and duration of involvement for the care team and coordinate with primary care providers.
  3. Use assessment tools to identify patients requiring care coordination.
  4. Conduct targeted outreach to high-risk patients to ensure timely care across the continuum.
  5. Manage populations using technological tools.
  6. Perform comprehensive assessments including disease-specific, age-specific, behavioral, and social needs.
  7. Communicate and share care plans with patients, families, and healthcare teams.
  8. Develop and monitor care plans collaboratively to maximize outcomes.
  9. Ensure care gaps are addressed around diseases and surgical episodes.
  10. Serve as the primary contact and advocate for patients, facilitating access to services.
  11. Coordinate care team members and transitions across settings.
  12. Partner with social work, pharmacy, palliative care, and other teams.
  13. Ensure compliance with disease-specific care paths.
  14. Assess patient knowledge and readiness, providing education and self-management support.
  15. Participate in quality and process improvement initiatives.
  16. In home care, oversee clinical staff and LPN teams to ensure appropriate services.
  17. Perform other duties as assigned.
Qualifications:
  • Graduate from an accredited nursing program; BSN preferred.
  • Current RN licensure; specialty certification preferred.
  • Basic Life Support (BLS) certification through AHA.
  • Experience: 3-5 years of nursing experience required.
Physical and Other Requirements:
  • Full range of motion, dexterity, and eye-hand coordination.
  • Corrected hearing and vision.
  • Exposure to communicable diseases or fluids possible.
  • Light work: exerting up to 20 pounds occasionally, 10 pounds frequently.
Protective Equipment:

Follows standard precautions and uses PPE as required.
