Job summary

Prisma Health Richland Hospital in Richland, SC is seeking an experienced RN to join Hospital Case Management. You will coordinate care transitions, collaborate with physicians and payors, and promote safe, cost-effective treatment plans that empower patients.

The role requires a BSN; an ADN or Nursing Diploma may be considered with a signed MOU. Must hold an active RN license; ACM/CCM is preferred. Day shift with strong teamwork and adherence to patient-centered care standards.

Qualifications

• Education: BSN preferred.
• Experience: 3 years acute care nursing; 1 year case management preferred.
• In lieu of BSN, ADN or Nursing Diploma may be considered if MOU signed to pursue BSN/MSN.
• Active RN license with NCSBN compact recognition or SC license.

Responsibilities

• Coordinate care transitions and post-acute plans.
• Collaborate with physicians, care teams and payors for efficient, quality care.
• Identify social determinants affecting outcomes and document plans.
• Ensure medical necessity and appropriate level of care.
• Navigate regulatory/compliance requirements and support discharge planning.
