Job summary

Central City Health in Detroit is seeking a Nurse Care Coordinator to be the clinical point of contact for an adult panel, ensuring continuous oversight across primary care, behavioral health, and related services. You will monitor care between visits, reconcile medications, and coordinate with pharmacists, case managers, and dental providers.

Qualifications

• Active, unrestricted Michigan RN licensure.
• BLS certification required.
• Strong clinical triage judgment and medication reconciliation competency.
• Minimum 2-3 years of clinical nursing experience; ambulatory/primary care or care coordination preferred.

Responsibilities

• Own a panel of adult CCV-eligible members at the site with ongoing coordination.
• Monitor the EHR for events between visits and coordinate with clinicians.
• Perform medication reconciliation and follow up after qualifying events.
• Coordinate hand-offs to Case Manager, Pharmacist, or Dental Provider as needed.
• Lead monthly multidisciplinary rounds and act as clinical contact between CCVs.
