Qualifications

Responsibilities

• Complete comprehensive assessments of members per timelines.
• Develop and implement care coordination plans with members and providers.
• Conduct telephonic, face-to-face, or home visits as needed.
• Monitor care plans to evaluate effectiveness over time.
• Maintain ongoing member caseload with regular outreach.
• Promote integration of services, including behavioral health and community resources.
• Facilitate interdisciplinary care team meetings and collaboration.
• Use motivational interviewing to educate and support members.
• Assess barriers to care and provide coordination assistance.
• May conduct medication reconciliation as needed.
