Job summary

Greater Regional Health seeks a skilled RN Care Coordinator to align team-based care for patients, families, and caregivers. You will coordinate referrals, develop care plans, and promote self-care in home settings, working across Med Surg, Diabetic Education, and ER as needed.

Ideal candidates have RN licensure with BSN within 3 years, experience in care coordination or home health, and strong communication to navigate community resources and health IT systems.

Qualifications

• RN with current licensure; BSN required within 3 years.
• Experience in care coordination or case management preferred.
• Ability to coordinate with patients, families, and providers.

Responsibilities

• Assess patient and family unmet health and social needs.
• Develop and monitor care plans with patient and provider.
• Maintain contact with family to report changes in health and goals.
• Promote health literacy and navigate community resources.
• Coordinate with outside organizations on legal matters as needed.
• Stay current on Federal and State rules and policies.
• Mobilize meetings and resources to achieve clinical outcomes.
