Senior Care Partners PACE in Kalamazoo, MI is seeking a licensed Registered Nurse to join our interdisciplinary team and provide in-home nursing care to program participants. You will conduct assessments, administer wound care and IV hydration, coordinate care with physicians and family members, and respond to urgent and emergent needs while supporting quality improvement initiatives.

Qualificações

• Must be a graduate of an accredited School of Nursing and licensed to practice in Michigan.
• 1–2 years of home care nursing experience; 5 years preferred, with frail or elderly populations.
• Strong clinical, communication, and critical thinking skills; proficient with MS Word and Outlook.

Responsabilidades

• Conduct participant assessments and determine level of care during home visits.
• Provide direct nursing care including wound care, IV hydration, and lab draws.
• Coordinate care with physicians, families, and IDT to develop holistic care plans.
• Respond to communications and triage concerns; document interactions.
• Participate in emergency response, follow-up care, and quality improvement initiatives.
