Job summary

Virginia Mason Franciscan Health is seeking a RN Care Coordinator to serve as a central figure in patient care, guiding the journey across the continuum. You will assess, plan, and facilitate comprehensive care, advocating for patients while collaborating with physicians, nursing, insurers, and post-acute providers to ensure timely, high-quality transitions.

Requirements include RN licensure in WA, 2+ years acute hospital experience, and BLS.

Qualifications

• Graduate of an accredited school of nursing.
• Minimum two (2) years of acute hospital clinical experience.
• Registered Nurse: WA.
• Basic Life Support - CPR within 90 days.
• Masters in Case Management or Nursing field (preferred)
• Bachelor of Science in Nursing (preferred)
• Certified Case Manager or Accredited Case Manager (preferred)

Responsibilities

• Completes and documents a discharge planning assessment on those patients identified by the designated screening process, or upon request.
• Facilitates the development of a multidisciplinary discharge plan with the patient, family, and post-acute providers.
• Oversees and evaluates the implementation of the discharge plan.
• Collaborates with the multidisciplinary team to ensure progression of care and appropriate inpatient resource utilization.
• Collaborates with post-acute providers to ensure timely and smooth transitions to appropriate post-acute services.
• Identifies readmission risk and implements interventions for at least a 30-day period.
