Job summary

Wellstar North Fulton Hospital is seeking an RN Care Coordinator for full-time day shift. The CC RN will assess transitional care needs, plan discharge, and coordinate with the care team to ensure smooth patient transitions.

The role requires an Associate's Degree in Nursing, GA RN license, and at least 1 year of acute care experience. Relocation assistance may be available for eligible candidates.

Qualifications

• Associate's Degree in Nursing required.
• Georgia RN license required.
• BLS certification required.
• Minimum 1 year nursing experience in acute care.
• Excellent written and verbal communication skills.
• Ability to work under stress in a fast-paced environment.

Responsibilities

• Assess transitional care needs and develop discharge plans.
• Coordinate care across the continuum with physicians and the care team.
• Facilitate interdisciplinary rounds and patient/family education.
• Identify and address barriers to discharge; engage social work when needed.
• Monitor length of stay and efficient utilization of resources.
