Qualifications

• RN license (single or multi-state) and BLS certification.
• At least 1 year of acute care nursing experience.
• Strong communication, assessment, organizational and problem-solving skills.

Responsibilities

• Conduct psychosocial and clinical assessments to inform discharge planning.
• Develop individualized discharge plans and facilitate care progression.
• Collaborate with physicians, care teams and community resources to promote timely discharges.
• Manage all aspects of discharge planning, including referrals and resource coordination.
• Document assessments, care plans and interactions accurately and promptly.
• Identify delays or obstacles in care and work to resolve them.
• Support ongoing professional development and mentoring peers or interns as appropriate.
