Job summary

Olathe Medical Pavilion is seeking an experienced RN Case Manager to coordinate care for patients across the hospitalization continuum. You will collaborate with the healthcare team to ensure safe, timely, and cost-effective discharge planning and post-hospital care, balancing patient needs with resource utilization.

The role emphasizes advocacy for patients, effective communication, and ongoing performance improvement within a multidisciplinary team.

Qualifications

• RN license and active state licensure in Kansas.
• Associate Degree in Nursing required; BSN preferred for hires after Sept 1, 2026.

Responsibilities

• Accepts responsibility and accountability for achievement of optimal outcomes within their scope of practice.
• Acquires and maintains knowledge and competence related to the expectations of their position and practices within their scope.
• Advocates on behalf of patients and caregivers for identification and access to services.
• Identifies and manages other patient needs making referrals to the appropriate personnel.
