Qualifications

Responsibilities

• Coordinate hospital and post-hospital care across care continuum.
• Collaborate with multidisciplinary team to plan discharge and post-discharge needs.
• Advocate for patient and family rights and ensure patient-centered care.
• Monitor utilization, cost, and quality outcomes for assigned cases.
• Maintain documentation in the Case Management system and participate in ICC huddles.
