Job summary

Virginia Mason Franciscan Health is seeking an RN Patient Care Coordinator to be a central figure in patient care, guiding the healthcare journey and ensuring smooth transitions across teams and settings. You will collaborate with physicians, nursing, insurers, and post-acute providers, assess and plan care, and advocate for patients to achieve timely, high-quality outcomes.

Applicants should be a registered nurse licensed in WA with at least two years in acute care, and BSN preferred; CPR/BLS

Qualifications

• RN required with WA license.
• Two years of nursing experience in an acute clinical care setting.
• Experience in Oncology, Med/Surg, Nutrition Support, Pediatrics, IV Therapy, Gerontology, and Home Health preferred.
• Basic Life Support (CPR) required.

Responsibilities

• Evaluates and admits patients to Hospice House service per procedures.
• Reviews admission information with patient and family.
• Evaluates and admits patients to Hospice service with interdisciplinary team.
