Benchmark Human Services is seeking a Registered Nurse (RN) to provide healthcare coordination and clinical oversight for individuals in the Crisis Support Home program in Savannah and Bloomingdale, GA. You will collaborate with mobile crisis teams and train unlicensed staff to ensure comprehensive health care and inclusion for our clients.

Responsibilities include overseeing medications, records, and transitions of care, while ensuring timely emergency responses and compliance with regulations.

Formación

• RN with current Georgia license.
• Experience in IDD/Mental Health nursing is preferred.
• Experience with individuals with intellectual, developmental disabilities and/or mental illness.
• Valid Driver's License, with no driving limitations.

Responsabilidades

• Provide healthcare and wellness coordination for individuals in accordance with state and federal regulations.
• Train unlicensed staff on medication supervision and treatment protocols.
• Oversee pharmacy and medication needs; maintain health-related records and assessments.
• Support transitions of care to/from hospital, nursing home, rehab facility and/or another agency.
• Respond to emergency medical situations promptly.
• Develop and update individual plans of care and health care plans.
