As a Care Coordinator - RN you will provide a deliberate patient and family centered partnership with the healthcare team by developing and supporting a coordinated plan of care between visits (Inpatient and Outpatient) to promote a smooth transition for patients and families across the healthcare continuum.

For 70 years, our Cardiovascular Thoracic Surgery program has led the way in heart care, transforming lives with major advances in open heart surgery, valve replacements, transplants, cardiac resuscitation, and other techniques to treat heart disease. Our team of surgeons, advanced practitioners, nurses, and healthcare professionals bring together a deep understanding of academic medicine and advanced clinical care to tailor our therapies to each patient's individual condition. The Cardiovascular Thoracic Surgery Team is looking to add a team member who values team collaboration and has a passion for helping others.

The Care Coordinator - RN works to maximize the patient's health outcomes and experience by identifying patient and family needs, completes pre-visit planning, supports clinic visits, and provides closure to the clinic visits. The Care Coordinator - RN utilizes coaching to help ensure patient and family participation in treatment plans and provides exceptional customer service to internal and external customers (providers, clinic staff, patients, and families).

What you will do as an RN Care Coordinator :

• Coordinates care across the health care continuum within designated sub-specialty population(s)

• Serves as a single point of contact by facilitating and coordinating transitions in care within and between care settings, providers, and services

• Completes pre-visit planning and post-visit wrap-up and determines vital follow-up appointments and/or treatments to optimize patient visits

• Facilitates proactive disease management and preventative health care using evidence-based practice

• Rooms patients for the surgeon(s) as needed, complete clinic readiness including records review and provides administrative support as needed.

What you will need:

• Current Minnesota RN license

• BLS certification

• Demonstrated experience in patient advocacy and education, coaching, and proactive health care management along with EMR/digital literacy

Location: 909 Fulton St. SE, Minneapolis MN

Hours: 40 hours a week
