Job Description

Job Title: Registered Nurse Well Care Coordinator

Location:

Remote (Evansville, Indiana) Must be an Indiana Resident (field visits required)

Hours:

Monday - Friday 8am-5pm

Pay:

$48 hourly | Weekly pay

Travel Requirement:

25% - 50%

Job Summary

The Community Well Care Coordinator oversees care coordination and complex case management for PathWays to Aging members living in the community who are not receiving Home and Community Based Services (HCBS) or are not at Nursing Facility Level of Care (NFLOC). The role requires a registered nurse or similar medical professional with extensive experience in caring for members aged 60 and above. The individual will work under the supervision of the Health Services Directors, managing care teams, developing and implementing care plans, and coordinating physical and behavioral health services. Collaboration with various stakeholders, including state staff, is essential.

Primary Responsibilities
  • Develop, mentor, and support staff within the department or region
  • Set goals, communicate expectations, and evaluate performance
  • Understand contractual requirements to meet members' needs
  • Identify and implement process improvements
  • Ensure standardized workflow processes, conduct audits, and ensure compliance
  • Assess, plan, and execute individualized care strategies
  • Refer members to social service programs
  • Communicate health-related information to stakeholders
  • Advocate for patients and families
  • Train and coach staff as needed
  • Conduct bi-annual field visits with direct reports
  • Collaborate with internal and external stakeholders
Required Qualifications
  • Resident of Indiana
  • BSN or equivalent experience
  • Unrestricted RN license in Indiana
  • Experience in community health care roles
  • Knowledge of Indiana Medicaid, Medicare, and Long-term care
  • Experience coaching or mentoring staff
  • Intermediate proficiency with Microsoft Word and Windows
Preferred Qualifications
  • 3+ years of healthcare case management experience
  • Background in managed care
  • Certified Case Manager (CCM)
  • Experience with long-term social supports
  • Experience with utilization review, concurrent review, or risk management
