Job summary

Grand River Health in Rifle, CO is seeking an Admissions/Discharge Case Manager (RN) to coordinate care from admission through discharge, working with providers, nursing staff, patients, families, and community partners to ensure safe, efficient transitions and high-quality outcomes.

The role requires current Colorado RN license, BLS, and ideally BSN with at least five years of nursing and case management or discharge planning experience; strong documentation, communication, and EHR skills are

Qualifications

• Current Colorado RN license (DORA) required; BLS certification required.
• BSN or equivalent preferred.
• Minimum 5 years nursing experience; case management or discharge planning experience favored.

Responsibilities

• Coordinate care plans, admissions, discharge planning, and transitions of care.
• Collaborate with providers, nursing staff, therapy, social services, outside agencies, and community resources.
• Assess patient needs, including social determinants of health, and connect patients and families with resources and support services.
• Monitor length of stay, discharge barriers, readmission risks, and opportunities to improve quality, efficiency, and continuity of care.
• Maintain accurate documentation, participate in care coordination rounds, and support compliance with hospital policies, regulatory requirements, and nursing standards.
• Performs a variety of tasks and other duties as assigned.
