Job summary
AdventHealth in Parker, Colorado is seeking an experienced Registered Nurse for discharge planning and care coordination. You will participate in multidisciplinary rounds, educate patients and families, assess readmissions, and organize care conferences to ensure timely transitions.
You will document evaluations, coordinate with payors for post‑acute authorization, and leverage EMR tools in a dynamic healthcare setting.
Qualifications
- RN license required
- Associates degree in nursing required
- Bachelor's degree preferred
- 2+ medical/hospital nursing experience required
- Care management/utilization management experience preferred
Responsibilities
- Participates in multidisciplinary rounds to review patient status, progression and discharge plans.
- Educates patients and families on illness impacts and mobilizes resources for discharge needs.
- Assesses readmissions and develops discharge plans with contingency planning.
- Documents discharge planning evaluations and communicates with payors for post-acute care authorization.
- Ensures timely care transitions and coordinates community care resources.
