Job summary

Wellstar Health System's Cobb Hospital is seeking an RN Care Coordinator for the Day shift to support patient transitions and discharge planning. The CC RN assesses psychosocial and clinical needs, coordinates care across the continuum with physicians, Nurses, social work, and case management, and educates patients and families to promote timely discharge.

Required: Associate's Degree in Nursing and Georgia RN license; minimum 1 year of acute care nursing experience.

Qualifications

• Associate's Degree in Nursing from an accredited program.
• Georgia RN license required.
• Minimum 1 year of nursing experience in acute care.
• Excellent written and verbal communication skills.
• Ability to work collaboratively with physicians and care teams.

Responsibilities

• Assess patients' chronic disease needs and psychosocial risk factors.
• Coordinate care across the continuum and with payers.
• Lead patient/family education and care conferences.
• Collaborate with physicians and care team for timely discharge.
• Document care plans and discharge plans in the medical record.
• Identify barriers to discharge and engage leadership for resolution.
