Carroll Lutheran Village in Westminster, MD is seeking a dedicated Full-Time RN Care Coordinator to join our mission-driven team. The role involves coordinating care for residents, educating families, and linking residents with community resources in a not-for-profit setting.
Key duties include developing care plans, following up after hospital stays, and serving as a clinical resource for Residential Living. RN license in Maryland and CPR certification are required; BSN is preferred.
We’re currently seeking a dedicated Full-Time RN Care Coordinator to become part of our caring and mission-driven community. This position is Monday through Friday, 8:00 AM to 4:00 PM, with on-call responsibilities as needed.
The RN Care Coordinator works closely with residents, families, and healthcare providers to coordinate care, provide education, and connect residents with resources that support healthy, independent living.
